FRONTLINE HEALTH SCREENER INFORMATION

FRONTLINE HEALTH SCREENER INFORMATION:
 
Parents or guardians must complete the Daily Frontline Screener each morning by 8:15am
(by 7:15 if the student has 0 period).
The Health Screener is required for Students to attend in person.   Link here.

Completing the screener by this time will ensure that your student is on the cleared list, which will speed up their entry process.

If your student has a cell phone, texting them a screenshot of their “You are Cleared” screen with student's name and date will also speed up their entry process. See examples below of screenshots you could share with your student to expedite the entry process.
 
It is ok if your student does not have a phone. We can check our list to be sure they are clear.

 
 
Daily Health Check for Students
As part of our community commitment to keeping each other safe, we are excited to announce our launch of the Frontline Health system which will make our daily health screening process accessible and efficient.  The system allows users to access the daily health screener through a website specific to Castro Valley USD (link here)
and/or through an app on your phone or other internet device.   App is called: Frontline Health Portal and has an icon like this: app icon

1. click the link titled “Activate Account”. as in image below:

2. For App users ONLY : Identify the state (California), the district (Castro Valley Unified School District) 
   
All users will then enter your email address into the blank box and select the blue tab Send me a temporary password”. 
    1. *Please note that you must use the email that we have on file in the Aeries system.  The Frontline Health Portal has created an account for you based on the information in Aeries and your student(s) cannot be linked to any other email address.
3. Check your email for a message
 
4. Once you receive the temporary password, please select the blue tab “I’m ready to Sign In”.
 
5.  Follow the prompts and then select the blue tab “Update my password”.
 
6. Review the terms, click the indicator “I have read and agree……”, and enter your name into the blank box.
*It is important that your name is written exactly as it appears in the statement “By entering my name……” which is written directly below the blank box.
 
7.  Please click the blue tabContinue to Assign People
 
8. Link all students to the account one at a time by following the prompts. Once you have added your student(s), please select the tab “I’m done adding students”. You will receive a “Your account has been activated” message.
 

Completing the Daily Screener on the app:


  1. Login and select the “Daily COVID-19 Health Check” from your to do list.
  2. Click anything that applies before selecting the blue tab “Submit”.
  3. After submitting the screener, a message will indicate if your student is  “Cleared” or “Not Cleared”. 
If you are completing additional screeners, click on the “Back” option at top left of the screen. Next click on the three line image at the top left of the home screen and select the “My People” option.
 
 

Completing the Daily Screener on the website (link here):


  1. Login to your account.
  2. Click the icon in the “Event” column on the very left of the “Daily COVID-19 Health Check”.
  3. Click anything that applies before selecting the blue tab “Submit”.  
  4. After submitting the screener, a message will indicate if your student is  “Cleared” or “Not Cleared”. 
 

Addressing Accidental Screener Failure in the Frontline Health Portal Website


  1. A message of “Health Check Not Cleared” means that your response triggered a safety concern.  If there is concern, stay home and make contact with your school. If the message is in error, you can immediately click the blue tab “Retake”.  
  2. Another option is, from the home screen, select the grey tab “History” which will show submitted screeners.
  3. Select the blue link “Delete” on the screener entry and then select “OK” to verify. 
  4. This will result in the daily screener reappearing on your home screen which you can open and retake
 

Addressing Accidental Screener Failure in the Frontline Health Portal Smartphone App


  1. Receiving a message of  “Health Check Not Cleared” means that your response has triggered a safety concern. If there is a concern, please stay home. 
  2. If this message is the result of error, click the “Back” option at the top left of the screen. 
  3. Next select the yellow tab  “Delete Today’s Health Check”.  The screener will reappear on your to do list so that you can retake it.
