REQUIRED FORMS:

  1. REQUIRED: Packet of papers from WOLF Schools Camp Arroyo. This packet includes Camp Arroyo's Behavior Management Policy and Health and Liability Form. This packet MUST be filled out and returned to your Advisory teacher by Friday, 11/21. 
  2. CVUSD Permission Form (click), DUE 11/21
  3. CVUSD Medical Forms, DUE 12/5, only as needed, must be signed by a physician. If you already have the following forms on file with our school, there is no need to submit them again. Any medication that is already held at Creekside will transfer with your student to Outdoor School. Please download and complete any of the following forms as pertains to your student, and submit them to our front office, or email them to our Health Office Technician, Mrs. Semonsen, tbraxtonsemonsen@cv.k12.ca.us no later than December 5th.
    -Authorization to Administer Medication Must be signed by physician (click)
    -Asthma Action Plan Must be signed by physician (click)
    -Severe Allergy Plan Must be signed by physician (click)

 

PAYMENT INFORMATION:

Payments for Outdoor School can be made at the front office. Checks are the preferred method of payment and can be made payable to “Creekside Middle School.” Credit card payments can be made through our webstore. We stronlgy discoruage sending large amounts of cash to school with your students.
