6th Grade Applications Accepted Feb. 29-March 13

2020-2021 OPEN Sixth Grade Enrollment

(for current 5th grade students residing in the Castro Valley Unified School District Boundary)

Applications for incoming 6th grade students were distributed at each elementary school. 

On Saturday, Feb. 29, Creekside and Canyon began accepting applications for all 5th grade students who reside in the Castro Valley Unified School District’s boundary. Parent/Guardian must turn the application and Statement  of Residency with supporting documents. Students are not required to be present to turn in the application. 

Applications should be turned in to the middle school office of choice between 8:30am and 3:30pm on school days March 2-13 with the latest being March 13, 2020 at 3:30PM. Please click here for the residency form website. Residency forms and supporting documents must be brought in to the school by the parent/guardian with residency documents and ID. 

The lines will move quickly if all parent/guardians have the following items completed in advance and ready to turn in:

6th grade application 2020-2021.pdf  or 6th grade application sp.pdf  or 6th grade application ch.pdf 

Statement of Residency English.pdf  or Statement of Residency Spanish.pdf or Statement of residency Chinese.pdf  if the parent/guardian has the PGE and one other bill in their name. Please bring ID. Please check the form for acceptable documents to prove residency.  

Shared Residency Form English.pdf or Shared Residency Form Spanish.pdf or Shared Residency Form Chinese.pdf if the PG&E is NOT in the parent/guardian name.  Both the person with the PGE in their name and the parent/guardian must come together to provide ID and documents as stated on the Shared Residency Form.  

If a 5th grade student does not turn in an application form by March 13th at 3:30PM, the student will be assigned to one of the middle schools based on space available. 

Space is not first come first serve. It is based on total applications received during the application period. As long as you turn in the application and residency verification with the required documents by the deadline stated on the application, your student will have the same chance as all other applicants to attend the school of choice. If either middle school exceeds their enrollment capacity, placement will be determined by lottery.  If needed, the lottery will take place on Monday, March 16, 2020 at 4:30pm in the CVUSD Board Room.  Please check the school's website on Monday, March 16th to see if a lottery will be held.
